A completed application packet consists of:
- Internship application form or ADA Standard Application Form
- Official transcripts
- Verification Statement of Completion of Dietetic Program or Declaration
of Intent to Complete Dietetic Program
- Letter of application (typed)
- Three sealed letters of recommendation
- Application fee of $65.00 payable to "Morrison's"
